FEES

We are participating providers with most insurance companies. Please refer to the section below to determine our provider status with specific plans. We will file your claim to your insurance company if we are participating providers. For patients with insurance plans we do not participate in, payment in full is due prior to leaving the office. For patients with insurance plans that we do accept, your insurance requires that we collect copayments and deductibles at the time services are rendered. An itemized receipt is available at any time upon request.
